- You are 18 years or older
- You live in one of the areas covered by the 37 health facilities in the 4 specified municipalities
- You have been newly diagnosed with high blood pressure or diabetes, or you already have a diagnosis with records
- You are not pregnant
- You do not have physical or verbal disabilities that make it hard to take part in group discussions or counseling
- You do not plan to move away within the next year
