This trial tests whether giving focused radiation to many cancer spots (SBRT) can work without using testosterone-blocking hormone therapy. It is for men whose prostate cancer has spread and is no longer responding to low testosterone, and who have already tried one of the newer hormone drugs.

Who can take part
- You are 18 or older and have a diagnosis of prostate cancer (adenocarcinoma type).
- Your cancer has spread to your bones or soft tissues and is seen on a PSMA PET scan.
- Your cancer is growing despite very low testosterone levels (castration-resistant).
- You have taken at least one newer hormone therapy (like abiraterone or enzalutamide) for 4 weeks or more.
- You have 10 or more cancer spots that can be treated with a precise type of radiation (SBRT), and no more than 20% of them are FDG-PET positive but PSMA-negative.
- You are in generally good health (ECOG 0-2) and have adequate organ function (blood counts, liver, kidneys).
