- Having heart surgery that uses a heart-lung machine (coronary bypass, aortic valve replacement, or both)
- Surgery is planned in advance, not an emergency
- No major kidney disease before surgery (no dialysis and creatinine below 2.0)
- Heart pumping strength (ejection fraction) is 40% or higher
