Who can take part
- You have been diagnosed with Stage C heart failure (a type of heart failure that requires ongoing treatment).
- You are 18 years or older and can read and speak English.
- You are currently taking a loop diuretic (water pill) for your heart failure.
- You have had a hospital stay for heart failure in the past 2 years, or you sometimes have difficulty managing your own care (like taking medications or recognizing symptoms).
- You do not have a mechanical heart pump, are not on dialysis, and do not have advanced cancer or a life expectancy less than 1 year.
