- You are 18 years or older and have had a kidney transplant
- Your physical function score (from a survey called SF-36) is 75 or lower at the time of your transplant
- You are able to walk for 6 minutes (with or without help)
- You can speak and read either Norwegian or English
- You are mentally and cognitively able to take part in the program and answer follow-up questions
