Who can take part
- You are 18 years or older
- You are a Native Hawaiian living in a participating homestead community
- You are overweight or have obesity (BMI of 25 or higher)
- You have been diagnosed with type 2 diabetes, pre-diabetes, high blood pressure, or high cholesterol (LDL over 130)
- You can do at least 150 minutes of moderate exercise each week, such as brisk walking
- You can read and speak English well
