  • You are 18 or older
  • You are a Native Hawaiian living in a participating homestead community
  • Your body mass index (BMI) is 25 or higher (overweight or obese)
  • You have been diagnosed with type 2 diabetes or prediabetes, high blood pressure, or high cholesterol
  • You can do at least 150 minutes of moderate exercise each week, like brisk walking
  • You are willing and able to join all parts of the study, including activities with your household
