- You are 18 years or older
- Your most recent heart imaging shows a pumping strength of 40% or less
- You have a clinic visit with an enrolled doctor within the next 7 days
- You are not already taking at least one of four key heart failure medication types (beta blockers, ARNI, aldosterone antagonists, or SGLT2 inhibitors) and have no allergy to that type
