Who can take part

  • You are 40 or older with a serious illness, or you are 65 or older.
  • You are Alaska Native and/or American Indian.
  • You have a primary care provider you see regularly.
  • You have had 2 or more visits with your primary care doctor in the past year.
  • You do not have advance directives already in your medical record.
  • You are able to take part in a 30-minute study visit by phone, video, or in person.
