Who can take part

  • You are between 40 and 80 years old.
  • You have been diagnosed with COPD and your lung function shows airflow blockage.
  • You were in the hospital for a COPD flare-up and are being discharged within 48 hours, or have seen a lung doctor within 14 days after leaving the hospital.
  • You are a current or former smoker (have smoked at least 100 cigarettes in your life).
  • You own a smartphone and can follow instructions to use a small sensor on your chest.
  • You can attend follow-up visits or telehealth appointments as needed.
