  • You are 18 years or older
  • You get all your primary and specialty care at this institution
  • You have been diagnosed with chronic obstructive pulmonary disease (COPD)
  • You have had at least two COPD flare-ups in the past year, which needed extra medication like antibiotics or steroids
  • You can provide informed consent (agree to join after understanding the study)
  • You do not have neuromuscular diseases or seizures
