Who can take part

  • You are 40 years or older.
  • You have had Type 2 diabetes for at least 5 years (whether or not you take insulin).
  • You have heart or blood vessel disease (like coronary artery disease, stroke, or peripheral artery disease) — OR — you have one of these diabetes complications: eye disease (retinopathy), kidney disease (low eGFR or protein in urine), nerve damage, or high inflammation (hs-CRP) along with at least two of: high blood pressure (for 6+ months), high LDL cholesterol, obesity, or smoking history.
  • You agree to wear a HEMOTAG chest sensor at home at least 3 days a week for 30 days at the start, at 6 months, and after any heart-related hospital stays.
  • You are willing to have standard clinic visits and blood tests (NT-proBNP).
  • You can provide signed consent and speak English.
