  • You have been diagnosed with heart failure (your doctor told you or it's in your medical records).
  • You have a recent test (within the last 2 years) that checked how well your heart pumps (called LVEF).
  • You have seen a doctor at Mass General Brigham in the last 2 years.
  • Your latest blood pressure is not too low (the top number is at least 90).
