  • You are 18 years or older.
  • You have been diagnosed with heart failure and were discharged from the hospital on prescribed medicines.
  • Your heart's pumping ability (ejection fraction) is 40% or less, measured by an ultrasound of the heart within the last month.
  • You can commit to follow-up visits and are willing to stay in the study group you are assigned to.
