Who can take part
- Be 5 to 18 years old
- Have type 1 diabetes for more than 12 months
- Currently have blood sugar that is not in goal range (for example, HbA1c above 8% or recent average glucose above 200 mg/dL)
- Plan to receive diabetes care at the University of California, Davis pediatric diabetes clinic during the next year
- Have reliable internet on a device that can do video and audio
- Live in California and be able to connect your home glucose device (CGM and/or meter, and insulin pump if used) to the internet device using Bluetooth or a cable
