- You are 60 years old or older.
- You speak English or Spanish.
- You have been diagnosed with asthma for more than one year.
- Your asthma is not well controlled.
- You do not have dementia.
- You do not have COPD, heart failure (stages 4-5), or other chronic lung diseases.
- You have smoked less than 15 pack-years (for example, one pack a day for 15 years).
