Who can take part

  • You are 40 years or older
  • You are a current or former smoker with at least a 10 pack-year smoking history
  • You have moderate to very severe COPD (GOLD stages 2, 3, or 4)
  • You have had 2 or more COPD flare-ups in the past year that needed antibiotics or steroids
  • You are able to use a smartphone or computer and go online at least once a day
