Who can take part
- You have heart failure with reduced ejection fraction (your heart pumps less than or equal to 40% of blood out each beat) and symptoms like shortness of breath or fatigue (NYHA class II–IV).
- You have COPD with at least moderate airflow limitation, confirmed by breathing tests (FEV1/FVC less than 0.7 and FEV1 less than 80% of expected).
- You are 18 years or older.
- You are not already taking a high dose of metoprolol (more than 23.75 mg per day) before the study starts.
- Your resting heart rate is at least 50 beats per minute, and you do not have certain heart rhythm problems like atrial fibrillation or heart block.
- You do not have severe liver or kidney disease, active asthma, or other serious conditions like cancer that would make the study unsafe.
