This study tests whether giving a bisphosphonate (a bone-strengthening drug) before parathyroid surgery helps protect bone health in people with primary hyperparathyroidism. It's for those who already have thin bones and are planning surgery.

Who can take part
- You are a postmenopausal woman or a man over 50 years old.
- You have been diagnosed with primary hyperparathyroidism (high calcium) and are planning to have surgery to remove the parathyroid gland.
- Your bone density is lower than normal (T-score of -1 or less) at the hip, thigh bone, or lower spine.
- Your vitamin D level is at least 50 nmol/l before starting the study.
- You do not have kidney problems (eGFR below 35), allergy to bisphosphonates, recent major dental surgery, or other conditions/medications that affect bone health.
