E3 Hypertension: A team program for blood pressure control

  • You are African American, Black, Latinx, or Hispanic
  • You have high blood pressure (stage 2) that is not controlled, with readings at 140/90 or higher
  • You see a primary care doctor at a participating Rush clinic
  • You have a smartphone (iPhone or Android) and an email address
