Who can take part

  • You are between 18 and 70 years old.
  • You get diabetes care at Cho Ray Hospital and plan to continue for at least six months.
  • You have diabetes and are taking diabetes medicine.
  • You were recently diagnosed with diabetes.
  • Your blood sugar (HbA1c) level is above 8%.
  • You use at least two insulin injections a day or just started a daily multiple insulin injection plan.
