Who can take part

  • You are 18 years or older.
  • You have been diagnosed with chronic heart failure for at least 6 months and your medications have been stable for at least 1 month.
  • Your heart failure symptoms are mild to moderate (NYHA class I, II, or III).
  • You have been in the hospital or had an outpatient visit requiring IV treatment for heart failure in the past year.
  • You or your caregiver can use digital tools like a smartphone or computer.
