  • You are 18 years old or older.
  • You have high blood sugar (diabetes), high blood pressure, or high cholesterol, OR you are a caregiver for someone who does.
  • You or the person you care for is currently taking medication for the condition that was prescribed by the Hospital Authority in Hong Kong.
