  • You are 30 years or older.
  • You have high blood pressure (above 140/90) or take medicine for it.
  • You are willing to check your own blood pressure and adjust your medication as needed.
  • You have a smartphone with internet and can use WhatsApp or Viber at least twice a week.
  • You can communicate clearly and don't have memory or thinking problems.
  • You plan to stay in the same home for the next 6 months.
