Who can take part

  • You are 18 or older and have been diagnosed with chronic heart failure.
  • You have been in the hospital or emergency room for heart failure within the past 12 months.
  • You have had trouble affording food or have financial instability, or you use Medicaid or both Medicaid and Medicare.
  • You prefer to speak English and have a working email address.
  • You live in North Carolina with a mailing address there.
  • You do not have severe kidney disease (on dialysis) or an LVAD or heart transplant.
