- You are 18 to 89 years old.
- You have been diagnosed with any type of heart failure since January 2023.
- You had a hospital stay within the past year.
- You have seen your primary care doctor or a heart specialist at least once in the past year.
- You live in the Greater East Bay or Greater Silicon Valley area of California.
- You do not have severe memory problems, end-stage kidney disease, active cancer that has spread, or active drug or alcohol use.
