This trial tests whether adding a precise, targeted form of radiation (called SBRT) to standard immunotherapy (atezolizumab and bevacizumab) can help people with advanced liver cancer that has spread to the blood vessels or outside the liver. You may be asked to participate in one of two treatment groups to see which approach works better.

Who can take part
- You must have a confirmed diagnosis of liver cancer (hepatocellular carcinoma).
- The cancer must have invaded the main blood vessels of the liver, or spread to other parts of the body.
- You can have no more than 5 tumors in the liver, and the total size of all tumors must be less than 20 centimeters.
- Your liver function must be good (Child-Pugh class A or B7), and you must not have had prior systemic therapy or radiation beads for this cancer.
- You must not have had prior immunotherapy for any cancer, and you must not have a history of bleeding in the gut within the last 6 months if you are to receive the standard immunotherapy combination.
- You must have adequate blood counts and kidney function, and be 18 years or older with an ECOG performance status of 0 to 2 (able to carry out daily activities).
