  • You are 75 years or older.
  • You have been diagnosed with frailty (feeling weak, slowed down, or losing weight without trying).
  • You have been diagnosed with malnutrition (not getting enough nutrients).
  • You are able to do basic daily tasks like bathing, dressing, or walking with some help (score of 60 or more on a scale of 0–100).
  • You do not have severe kidney disease or a terminal illness.
