Who can take part
- You are 18 or older.
- You have high-risk prostate cancer that has not spread far (stage cT3/4 or high Gleason score or high PSA).
- Your PSMA test shows strong uptake (confirmed by PROMISE V2 criteria).
- You are well enough for daily activities (ECOG 0-1) and expected to live more than 10 years.
- Your blood counts, kidney function, and albumin are within safe ranges.
- You have no distant metastases.
