Who can take part
- You are 18 to 80 years old.
- You have had type 2 diabetes for at least 3 months.
- You have had heart failure with reduced pumping function (HFrEF or HFmrEF) for at least 3 months.
- Your heart failure symptoms are stable, and you have been on the same heart failure and diabetes medications for at least 2 weeks.
- You have not been hospitalized for heart failure in the last 4 weeks, and you do not have severe kidney disease, serious liver problems, or other major health issues that would limit life expectancy to less than 3 years.
