- You can sign a consent form and can follow the study for its full duration
- You’re age 30 to 70 and your BMI is 27.5 to 40
- Your type 2 diabetes labs (HbA1c) are between 6.5 and 10
- Your kidney function is adequate (eGFR above 45), and your blood pressure is not over 145/65
- You have HFpEF (heart failure with preserved pumping, ejection fraction over 50%) and NYHA class II to III heart symptoms
- You are on an approved diabetes treatment (diet/exercise, metformin, sulfonylurea, metformin+SU, DPP-4 inhibitor, or insulin)
