Who can take part
- You are between 16 and 19 years old
- You have had type 1 diabetes for at least 12 months
- Your recent A1C (average blood sugar level) was 7.0% or higher
- You currently receive diabetes care at a Seattle Children's Hospital clinic in Bellevue, Everett, Federal Way, or Seattle
- You can fill out written surveys
- You can receive diabetes care in Washington State for the next 2 years
