This study tests whether using an AI system can help community doctors better manage patients with high blood pressure along with either diabetes or high cholesterol. If you have uncontrolled high blood pressure and also have diabetes or high cholesterol, you may be eligible.

Who can take part
- You are 40 years or older
- Your blood pressure is high: top number 140 or higher or bottom number 90 or higher
- You have diabetes with HbA1c of 7.0% or higher, or high cholesterol with LDL 3.4 mmol/L or higher
- You can use a smartphone on your own or with help from family
- You have lived in a participating community for at least 6 months and plan to stay for the next 3 years
- You are enrolled in basic medical insurance
