Who can take part

  • You were 18 or older when diagnosed with high blood pressure
  • You have a diagnosis of high blood pressure, or two high readings (140/90 or higher) within one month
  • You have been followed by the same clinic for at least 6 months after your first diagnosis or high reading
  • Your doctor uses an electronic medical record system with your visits, diagnoses, and prescriptions
