  • You attend kidney (nephrology) clinics at Guy's and St Thomas' Hospital
  • You have advanced kidney disease (stage 5 CKD)
  • You are either on dialysis or nearing the need for dialysis
  • You have been referred to see a kidney transplant surgeon for evaluation
