Who can take part
- You have been diagnosed with high blood pressure (hypertension).
- You have had at least two blood pressure readings of 130/80 or higher (even if you take medication).
- You are not already enrolled in the OSF Saint Francis Medical Center Hypertension Center.
- You are not currently receiving palliative care or hospice care.
- You are not pregnant.
- You do not live in a nursing home and do not have end-stage kidney disease.
