- You are an adult (18 years or older)
- You have been diagnosed with high blood pressure (hypertension)
- Your high blood pressure is not caused by another condition (like kidney disease or a hormone problem)
- You do not have a terminal (life-limiting) illness
- Your high blood pressure is not related to pregnancy
- You do not have significant memory or thinking problems (cognitive impairment)
- You can see well enough to read a blood pressure monitor and use a computer or phone for health messages
- You do not have a psychotic disorder (like schizophrenia)
