Who can take part

  • You are 65 years or older.
  • You receive care at a UNC Health or UNC Physicians Network clinic.
  • You use insulin at least once a day.
  • You have not used a continuous glucose monitor (CGM) in the past year.
  • You are willing to use a smartphone to see your glucose readings.
  • You have a care partner (like a family member or friend) who lives with you and is 18 or older.
  • You and your care partner can attend sessions for 6 weeks and speak English.
