Who can take part
- You are 65 years or older.
- You receive care at a UNC Health or UNC Physicians Network clinic.
- You use insulin at least once a day.
- You have not used a continuous glucose monitor (CGM) in the past year.
- You are willing to use a smartphone to see your glucose readings.
- You have a care partner (like a family member or friend) who lives with you and is 18 or older.
- You and your care partner can attend sessions for 6 weeks and speak English.
