- You are 18 or older
- You were on a breathing machine for more than 3 days
- You stayed in the ICU for more than 7 days
- You are considered high risk based on a tool that looks at things like recent hospital stays, multiple health problems, many medicines, mental health history, and living situation
- You are expected to leave the hospital in the next few days
- You are not in the ICU mainly due to a brain or nerve condition, are not in hospice care, and you could walk independently before this hospital stay
