  • You were between 18 and 65 years old when you were admitted to the ICU
  • You stayed in the ICU for at least 48 hours
  • You left the ICU within the last 2 years
  • You have diabetes, chronic heart failure, or coronary artery disease
  • You have a smartphone and internet at home
  • You do not have a pacemaker or defibrillator implanted
