This registry is for people who may have a genetic form of diabetes (monogenic diabetes) diagnosed at a young age. It collects information to better understand and diagnose these rare diabetes types.

Who can take part
- You were diagnosed with diabetes at age 25 or younger.
- Or you were diagnosed between ages 26 and 35 and have a parent, sibling, or child who was also diagnosed with diabetes before age 40.
- Your body mass index (BMI) at diagnosis was 28 or lower.
- You do not have a positive test for GAD antibodies (a marker for type 1 diabetes).
- Your diabetes is not caused by another condition (like pancreatitis or certain medications).
- You have not had cancer (other than some skin cancers) in the past 5 years.
