  • Advanced Parkinson's disease (Hoehn & Yahr stage 4–5) diagnosed at least 5 years ago
  • Currently taking levodopa (a Parkinson's medication) with good response for at least 12 months
  • Moderate-to-severe motor symptoms or significant medication side effects despite treatment
  • Stable on current medications for at least 4 weeks before enrollment
  • Able to commit to 12 months of follow-up visits and no other brain surgery during that time
