Who can take part

  • You are 18 years or older
  • You have high blood pressure (systolic reading of 130 or higher on at least 2 visits about 1 week apart)
  • You have diabetes or pre-diabetes (hemoglobin A1c of 6.5% or higher—a blood test that shows average sugar levels)
  • You live in the health center's service area and are willing to receive care there
  • You are not pregnant and do not plan to become pregnant in the next 2 years
  • You are able to visit the health center for study visits over the next 2 years
