Who can take part
- You are 18 years or older
- You have high blood pressure (systolic reading of 130 or higher on at least 2 visits about 1 week apart)
- You have diabetes or pre-diabetes (hemoglobin A1c of 6.5% or higher—a blood test that shows average sugar levels)
- You live in the health center's service area and are willing to receive care there
- You are not pregnant and do not plan to become pregnant in the next 2 years
- You are able to visit the health center for study visits over the next 2 years
