  • You are 30 years or older
  • You have high blood pressure (140/90 mmHg or higher) or are taking medication for it
  • You have high blood sugar (HbA1c of 6.5% or higher) or are taking medication for diabetes
  • You are able to agree to join the study (give informed consent)
  • You do not have a disability that would make it hard to take part
  • You are not pregnant or have a severe illness that keeps you in bed
