- You are 30 years or older
- You have high blood pressure (140/90 mmHg or higher) or are taking medication for it
- You have high blood sugar (HbA1c of 6.5% or higher) or are taking medication for diabetes

- You are not pregnant or have a severe illness that keeps you in bed
