- Age 18–65 years old
- Imaging (CT, MRI, or angiography) confirms a blood clot in at least one major vein in the brain, diagnosed within the past 21 days
- At least one serious symptom or complication: severe headache, seizures, weakness, confusion, or bleeding in the brain
- Brain MRI shows the clot is in an early (acute or subacute) stage, not chronic
- No severe brain swelling with life-threatening midline shift requiring emergency surgery
- No active bleeding disorder, severe contrast allergy, or pregnancy
