Who can take part

  • You had a stroke caused by a blood clot (ischemic stroke) and have been discharged from the hospital
  • Your stroke occurred within the last 3 months
  • You have difficulty swallowing (dysphagia) that ranges from mild to moderate-to-severe
  • Your vital signs are stable and you are not bedridden or requiring intensive monitoring
  • You own a smartphone and can use it to receive care and support
  • You plan to live in the local area for at least the next year
