  • You are 18 or older
  • You had a recent stroke or transient ischemic attack (TIA) within the last 2 weeks
  • You have high blood pressure, diabetes, or both
  • You or a family member can use a smartphone to follow the study tasks after you leave the hospital
  • You are not already in another stroke study, and you are not pregnant or breastfeeding
