  • You are 40 years old or older
  • You have been diagnosed with stage 1 or 2 cardiovascular-kidney-metabolic (CKM) syndrome—a condition affecting your heart, kidneys, and metabolism
  • You receive regular outpatient care (visiting a doctor's clinic, not hospitalized)
  • You can speak and read Mandarin or Taiwanese
  • You own or have regular access to a smartphone or tablet
