Who can take part

  • You are between 45 and 59 years old.
  • You have been diagnosed with frailty or are showing early signs of it.
  • You have early-stage heart-kidney-metabolic syndrome (Stage 1 or 2), as defined by the American Heart Association.
  • You can communicate normally and use a smartphone or wearable activity tracker.
  • You are willing to fill out surveys and come back for follow-up visits.
