Who can take part
- You are 18 years old or older
- You have been diagnosed with diabetes or have high blood sugar levels (fasting glucose >126 mg/dL, random glucose >200 mg/dL, or hemoglobin A1c >6.5%)
- You are currently taking medication to lower blood sugar, or have symptoms of high blood sugar
- You are not currently in prison or detained
- You live in Nigeria or have access to participating clinics
