Who can take part
- You must be 18 years old or older.
- You must have a previous diagnosis of diabetes.
- You must have high blood sugar readings on at least two separate tests (random blood sugar over 200 mg/dL, fasting blood sugar over 126 mg/dL, or A1c over 6.5%), if those tests are available.
- You must be taking medication to lower blood sugar.
- Pregnant women and adults with cognitive impairment are also welcome to join.
