  • You are 18 years old or older
  • You have been diagnosed with diabetes or have high blood sugar levels (fasting glucose >126 mg/dL, random glucose >200 mg/dL, or hemoglobin A1c >6.5%)
  • You are currently taking medication to lower blood sugar, or have symptoms of high blood sugar
  • You are not currently in prison or detained
  • You live in Nigeria or have access to participating clinics
